Therapy Terms & Conditions

My signature below confirms that I understand that I can expect total confidentiality apart from the following: 

• Intent to significantly harm self or others / act of harm to self or others.
• Disclosure of unreported abuse on a young person below the age of 18, or on a vulnerable adult. 
• Suspected involvement in an act of terrorism, drug trafficking or money laundering. 
• Liaison with health professionals as part of a coordinated care package. 
• Court order. 
• (Under 18’s) The young person requests, and feels that it is within their best interest, that certain information be passed on. 

 

I, the client, Agree to the following conditions: 

• Individual Sessions will last 50 minutes and will begin and end at the agreed times. 
• Couples Sessions will last 60 minutes and will begin and end at the agreed times. 
• Clients referred to me, by specific companies with a pre-arranged agreement, are entitled to 3 to 6 sessions paid for by the company.
• (Individual Client) If I am late by more than 10 minutes my session will be cancelled, the fee will still be payable, and it is at my counsellor’s discretion whether she offers a rescheduled appointment or not.
• (Couples) If I am late by more than 20 minutes my session will be cancelled, the fee will still be payable, and it is the counsellor’s discretion whether she will offer a rescheduled appointment. 
• (Couples Therapy) The session will start when BOTH parties are present.
• If accessing counselling via Zoom or any other social platform I will not record the sessions or have any other people present, I will be in a safe & confidential environment and use headphones if necessary.
• Payment to be made the day before the session as this reserves your space, if no payment is made then the session will not take place and the space will be offered to someone else.
• If I persist in cancelling or deferring payment, then the appointment time will be given to another client with immediate effect. 
• If I need to cancel or reschedule an appointment, I will inform my Counsellor as far in advance as possible, and at least 24 hours before my scheduled session. 
• That, if I do not attend a session, having given less than 24 hours’ notice or no notice at all, I am liable for the full session fee.  
• That it is my counsellor's discretion whether she offers a rescheduled appointment or not. 
• That I will not be under the influence of drugs or alcohol in session. 
• To inform my counsellor of any changes of contact details/address. 
• That I will Pay an administration Fee of £25 per legal document in regard to communicating with “Authorized specific consent by the client” to third parties. 
• I understand that if I pay concession fees, these will be reviewed every 3 months. 
• I understand that if I pay full rate session fees, these will be reviewed Bi-annually in accordance with the payment policy. 

 

Your Counsellor Agrees to the following conditions: 

• Couples Sessions will last 60 minutes and will begin and end at the agreed times. 
• Individual Sessions will last 50 Minutes and will begin and end at the agreed times.
• If offering counselling via Zoom or any other social platform, I will wear headphones and be in a safe confidential environment. 
• To give as much notice to you as possible, and at least 24 hours’ notice, in rare instances where I the Counsellor needs to cancel or reschedule the session. 
• To offer counselling in accordance with the COSRT and BACP Ethical Framework.
